What the coverage includes
Your SFUO Health Plan covers prescription medications at 80%. So for each prescription all you pay is a co-payment of 20% plus the dispensing fee minus $5. The maximum amount that you can be reimbursed is $1,500 per policy year.
- Oral contraceptives are covered up to a maximum of $200 per year.
- Vaccines are covered up to a maximum $200 per year.
- Unless your doctor specifically prescription a brand name medication, a generic equivalent drug will be given as a substitution (a refund will be based on the lowest priced generic equivalent). Needles and syringes are covered when prescribed by a doctor.
How do I get my benefits?
When you go to the pharmacy you have two options for claiming:
Option 1: You can print out your Health Plan Card. Write your name and student number on the card and fold it as indicated. When you go to the pharmacy, show this card to the pharmacist and they will process your claim automatically. This will allow you to simply pay the difference.
Option 2: You can pay full price for your prescription and then fill out a drug claim form. Once you complete the claim form you will send the original receipts and the claim form to the following address:

Please keep a copy of all documents that you will send to the insurer for at least a year. If you do not have the original receipts, you can ask your pharmacy to reprint them (some pharmacies charge for reprinting). Please make sure that the address you put on your claim form is a valid address because your refund will be mailed to that address. Your refund could take a few weeks.
