Coverage: Vision Coverage
What the coverage includes
Your SFUO Health Plan covers $150 of vision care per 24 consecutive months.
Eligible Expenses:
When prescription lenses are first prescribed or when required by a change in prescription.
If your eye glasses have been lost, stolen, broken or replacement is required provided you have been insured for at least two years and at least 24 consecutive months have elapsed since you last claimed benefits under the lenses and frames or contact lenses provision.
When contact lenses are medically necessary to correct severe astigmatism, severe corneal scarring, keratoconus or aphakia, provided sight can be improved to at least the 20/40 level. Under these circumstances, the maximum eligible expenses for vision care are replaced by a maximum of $200 per period of 24 consecutive months. However this will be decided upon by the Insurance company.
