Who is the insurance provider?
Assumption Life is the insurance provider for the SFUO Health Plan’s medication, dental and extended health care coverage.
What are my group number and certificate number?
Your group number is 1480 (this is the same for all uOttawa undergraduates), your certificate number is your student number followed by the letters OTT (ex. 3217654-OTT).
How long does my coverage last?
The policy year is from September 1st to August 31st so your SFUO Health Plan has got you covered throughout the summer as well.
How does my Health Card work?
Download your Health Card. Fill in your name and student number. You can bring the Health Card to almost any Pharmacy and show it to get your benefits right away.
Can I claim my insurance if I didn’t use my Health Card?
You can always submit your original receipts by mail. You must attach your receipts to the claim form that you can download and print in the downloads section. It is strongly recommended that you keep a copy of all documents that you will send to the insurer. If you do not have the original receipts, you can ask your pharmacy to reprint them. Ensure that you indicate a valid address because the refund cheque will be sent to this address.
What is the non-coverage period?
At the beginning of the year, there is a period when the list must be updated. During this period, students who were not on the list for the previous year will not have access to payment with the Health Card.
My pharmacy/dentist told me that my coverage does not work or is expired.

Does the insurance cover me outside Canada?
Yes, we have out of province and out-of-country coverage, but medication should be bought in Canada. Visit the travel insurance page for more details. You can also contact Assumption Life customer service at 1-888-869-9797.
