Registering yourself or your family
How to register
Part-time students and full-time student who register in the winter are not automatically members of the SFUO Health Plan. If you would like to opt-in to the plan you can fill out the required registration form and pay your premiums at the SFUO office (Room 07, Jock-Turcot University Center).
- Part-time students: The deadline to register and pay is October 15th, 2009. Premiums are $360 for Health and Dental coverage.
- Winter full-time students: The deadline to register and pay is February 1st, 2010. Premiums are $180 for Health and Dental coverage.
How to register your family
You have the option of providing your family with the same benefits you enjoy as a member of the SFUO Health plan. If you would like to register your family under the plan you can fill out the required registration form and pay your premiums at the SFUO office (Room 07, Jock-Turcot University Center).
- Family coverage: The deadline to register and pay is October 15th, 2009. Premiums are $540 in addition to your $180 premium as a full-time student or $360 premium as a part-time student.
- Spousal coverage: The deadline to register and pay is October 15th, 2009. Premiums are $360 in addition to your $180 premium as a full-time student or $360 premium as a part-time student.
- Single parent family coverage (2 children): The deadline to register and pay is October 15th, 2009. Premiums are $180 in addition to your $180 premium as a full-time student or $360 premium as a part-time student.
